Any other empaths out there having similar experiences?

I believe it, that would be the easiest way, especially for the elderly patients. That's what they do at hospice.

Because fentanyl wouldn't show up in an autopsy

Not unless you look for it specifically. An autopsy of an elderly person from a nursing home is not a murder investigation, so they wouldn't test for it.
