I would think that Schiff gave the kid additional drugs, while the kid's handler had also higly drugged him and that along with the brutal sex act probably caused the death. It is unknown or proven at this time, but it is known that the victims are often gagged and tied down, which sometimes leads to asphyxiation, especially if drugs are involved.

I lived in Los Angeles in the height of the Paris Hilton clubbing era... I went out a lot. Like 5 nights a week a lot... There were ALWAYS "creepers" in EVERY bar.
