"The audit said the agency did not consider sexual abuse as something that rose to the level of "critical incident," and that's why agency officials did not report those 118 cases to the Office of the Child Advocate during the period of the audit.
The agency is also not ensuring that potentially criminal actions are referred to prosecutors for investigation and potential legal action, the audit said.
During the two-year period of the audit, the agency was also unaware of 260 incidents "of what appeared to be serious bodily injury to children in its care," Bump's office said.
