Looks like there is a white bag with pastries of some sort in it. Good eye. Least this has some rational explanation (even if the kid looked drugged, could just have been snap timing for that part). Only thing left there IMO is what looks like bloodshot eyes on the cartoon face. Looks too spaced out to just be cracks in a fading shirt design, but that's just MHO.
yeah.. looks like a donut with the same purple color jam across the top of it.. not sure what's up with the crazy red-eye elmo shirt.. the shirt the adult is wearing is a creepy band tho..
As far as "crazed Elmo" goes, the screen printed image has been worn and cracked due to wear and tear and the clothes dryer allowing some of the red tshirts to show through appearing as "bloodshot eyes".
"crazy red-eyed elmo shirt"... i scrutinized it.. it's not possible to tell it's simply wash-wear, but even so.. i don't think it's cause for alarm. it was just a pic on one of the 4chan posts about GF.

purple is the color of the jam... typically, grape jam is purple... donut.. like the one on the table behind the kid in the picture...
