She does have a more masculine face, but when I was a kid in the 90s I grew up around some Muslim people and a lot of the females had that look. I think it's an unfortunate combination of weight distribution (carb rich diet, because rice is cheap or free from the food bank), not being allowed to wear makeup or even pluck the eyebrows, stress from doing all the housework, plus a lot of them are refugees. Stress hormones in pregnancy and childhood act in a masculinizing way.
Also, as a Muslim person, why would they want to be female? Like even if they feel female, wouldn't they prefer all the perks they get as a dude compared to being able to "live authentically" as a doormat?

As long as you're using "gender" instead of the correct term SEX you are playing their game. They have won.
